Chapter 9 - The Safehouse

The safehouse was a modest, two-story brick home tucked away in the quiet, industrial outskirts of Astoria.

Inside, the living room had been completely stripped of furniture, replaced by a clean, sterile field setup that Dante’s men had prepared in advance. It was a makeshift clinic, complete with surgical lighting, a ventilator, and three refrigerators filled with emergency medical supplies.

They carried Luca inside on a folding stretcher. He was barely conscious, his eyes rolling back, his skin the color of old parchment.

"Get the blood!" Elena yelled, her hands flying over his central line port. "I need the rapid infuser! Now!"

Dante and his men worked under her direction like a well-oiled machine. They hung the bags of O-negative blood, connected the heating lines, and began pumping the life-saving fluid into Luca’s veins.

Elena knelt beside the gurney, her stethoscope pressed to his chest.

"His left lung is still clear," she muttered to herself. "But his abdomen is distending. He’s bleeding into his peritoneal cavity. I need to find the source."

She looked at Dante. "Do you have an ultrasound machine?"

"Yes," Dante said, running to a closet and pulling out a portable, tablet-sized ultrasound unit. "We took it from the private clinic last month. Is it enough?"

Elena grabbed the probe, covered it in sterile gel, and pressed it against Luca’s left upper quadrant.

The black-and-white screen of the tablet lit up. Elena’s eyes scanned the gray structures of his liver and spleen. There, tucked beneath the splenic capsule, was a large, dark pool of free fluid.

"The suture on the main branch is holding," Elena breathed, her relief short-lived. "But there’s a small, accessory splenic artery that Dr. Chen missed. It’s leaking. It’s a slow bleed, but in his state, it’s fatal."

"Can you fix it?" Dante asked.

"I’m a nurse, Dante," Elena said, her voice cracking. "I don't do vascular surgery. I don't have the instruments, and I don't have the training."

"You have to," a voice whispered from the gurney.

Luca was looking at her, his eyes glassy but filled with an unshakeable, terrifying trust. He reached up, his cold, trembling hand grabbing her wrist.

"I won't let anyone else touch me, Elena," Luca whispered. "You do it. Or I die here."

Elena looked at his hand on her wrist. She looked at Dante, who was looking at her with the same absolute certainty.

She took a deep, shaky breath, her mind racing through every surgery she had ever witnessed, every anatomical chart she had ever memorized during her late-night study sessions in nursing school.

"Get me the surgical tray," Elena said. "The one with the micro-vascular clamps and the 5-0 prolene sutures. And get me a bottle of epinephrine."

She washed her hands with surgical scrub, pulled on sterile gloves, and knelt over the man who held the fate of the city in his hands.
